Institutional & Memorial (I&M) Grant Application Fall 2013 ~ Grant Choices (PDF document)
These Grants are provided by OCC alumni and other individuals in memory of a loved one. You must apply for I&M Grants each semester. Deadlines: Fall semester April 1st / Spring semeter November 1st

Other Financial Aid Forms
Financial Aid Authorization -- Gives the Financial Aid Office authorization to discuss your financial aid with others (i.e. parents, spouse, clergy, etc.)
Credit Balance Authorization -- Authorizes the Business Office to hold a credit balance on your student account for next semester.
Student Payment Authorization --Authorize the Business Office to use Federal Financial Aid to pay for other charges such as bookstore balances.
Loan Change Form -- Make changes to your Stafford Student Loan.
