Student Concerns
It
is the desire of the faculty, staff, and administration that the community
environment be conducive to student’s academic and personal growth.
Therefore, input from students is greatly encouraged. Students are encouraged
to submit their specific suggestions, concerns, or complaints regarding their
experience at OCC. These suggestions can be submitted to the Dean of Students
by completing the “Student Concern Form”, available below or in
the Student Development Office located in the Casteel Administration Building.
Students may also choose to voice their concerns directly to the appropriate
individuals either in writing or in person.
INSTRUCTIONS FOR THE STUDENT CONCERN FORM
Students who have specific suggestions, concerns, ideas, or complaints regarding
their experience as a student at OCC are encouraged to use this form to make
those known to the appropriate persons or department. Completed "Student
Concern Forms" will be reviewed by the Dean of Students to determine how the
concern should be addressed.
Concerns will be forwarded to the appropriate individuals or departments
for their consideration and response. For instance, concerns relating
to academic matters will be forwarded to the Academic Dean.
The student will receive a written or personal response from the Dean of
Students within two weeks of the date the form is submitted. This will
give either an answer to the concern or the measures being taken to address
the concern.
Please fill this form out completely using the following instructions.
- Mark your specific area of concern.
- Provide a detailed explanation of your concern written legibly in the
space provided or on an attached page. (If you use attached pages
please use the "Student Concern Form" as a cover page.) One issue
per form please.
- Be sure to include your name and OCC mailbox number on the form and any
attached pages. Anonymous forms will not be processed.
You may use the Online
form or complete a printed
copy (PDF). Completed paper forms should be submitted in the
Student Development Office located Casteel Administration Building.
Any questions related to the completion or use of the "Student Concern Form"
should be directed to the Dean of Students.
