How to Apply: Returning Student

Former students who have been absent from OCC for one semester or more

Students who have been absent from OCC for one semester:

• Reactivation Form (Online  or PDF)
• $10 Non-Refundable Processing Fee 
• Transcripts from any colleges attended while away from Ozark 
• Transfer Applicant Evaluation (TAE) from other colleges attended (Filled out by the Dean of Students of the college)  

Students who have been absent from OCC for two semesters or more:

• Reactivation Form (Online or PDF) 
• OCC Application (Apply Online  or PDF )
• $10 Non-Refundable Processing Fee
• Minister/Church Leader Reference  (PDF) 
•  Employer/Teacher Reference (PDF) 
• Transcripts from any colleges attended while away from Ozark
• Transfer Applicant Evaluation (TAE) from other colleges attended (Filled out by the Dean of Students of the college)
  
  The deadline for reactivating is August 5 (Fall) and January 5 (Spring). Any reactivation received after this must automatically late enroll and pay the late enrollment fee. If you have any questions, please feel free to contact us at 1-800-299-4622.
